Authorization for Medication
Print and sign the Authorization for Medication form HERE. Bring first day of camp.
AKASPORT will only administer physician prescribed medications. All medications must be in the original containers with the child’s name, name of the drug and directions for its administration and storage on the label. AKASPORT will keep a written record of the administration of any medication which will include the time and date of each administration, dosage, name of the staff member administering the medication and the name of the child. All medication disbursement will be supervised by AKA staff. AKASPORT staff will store all medications under proper conditions for sanitation, preservation, security, and safety. All unused medication will be returned to the parent/guardian at the end of each camp.
